A program to improve the health of rural communities through collaborative innovation.

The mission of the Rural Health Education Network of Schoharie, Otsego, and Montgomery Counties (RHENSOM) is to improve the health of rural communities through collaborative innovation, by implementing and facilitating evidence-based programming and best practices.

Current Goals & Objectives:

  • Distributing Chronic Disease Self-Management Toolkits
  • Facilitating the National Diabetes Prevention Program
  • Facilitating the CDC’s Eat Healthy, Be Active 6-week program
  • Organizing Hospital-based Farmer’s Market
  • Coordinating Worksite Community Supported Agriculture (CSA) programs
  • Collaborating to provide Mental Health First Aid trainings
  • Funding Community-based Nutrition Classes offered by Cornell Cooperative Extension (CCE)
  • Increase access to evidence-based programming for older adults by providing Schoharie County Council of Senior Citizens, Inc. and Otsego County Council of Senior Citizens Inc. with scholarships.

History:

Funded by New York State, Charles D. Cook Office of Rural Health, Rural Health Network Development program since 1997, our network has provided numerous health education activities and programs.
